2. Understand how KeepWell, an eHealth self-management tool was developed and evaluated,
3. Discuss the implication of findings, including the feasibility of conducting an effectiveness-implementation hybrid trial.
Dr. Monika Kastner is the Research Chair in Knowledge Translation and Implementation at North York General Hospital, and Associate Professor at the University of Toronto in the Institute of Health Policy, Management and Evaluation and the Department of Family and Community Medicine. She is also the lead of the Diabetes Action Canada Knowledge Mobilization and Implementation Science (KM-IS) Program. Monika’s goals are to improve the health of priority populations and to advance the science and practice of KM-IS. As a health services and systems researcher, her work focuses on improving the health of priority older adults across diverse intersecting identities (e.g., sex, gender, race) who live with multiple chronic conditions and social frailty, using innovative e-health technologies that optimize healthcare delivery, clinical decision-making, and improved patient outcomes.
